COLLECTION OF PERSONAL INFORMATION
To access healthcare from Oxford Medical Centre, you must first complete a brief registration form. During registration you are required to provide certain personal information (such as name, address, gender, contact number, and email). By registering, you agree to receive certain communications from Oxford Medical Centre in concordance with your health.
USE OF PERSONAL INFORMATION
Personal information collected through our registration is used by us to, among other things, process and manage your records, operate and improve our database, track revisits and your feedback, make revisits easier for you by eliminating the need for you to repeatedly registering with the same information, and for other purposes pursuant to your consent. We also use your personal information to communicate with you in other ways. For instance, we may use your personal information to contact you about an upcoming appointment or to uplift laboratory results. We also use personal information to inform you of significant changes to this Privacy Policy. Clinical history is used by physicians to better evaluate, diagnose and provide relevant treatment.

We may provide personal information to other Physicians who would require said information to assist in diagnosing or establishing treatment which would benefit you. We may access, use or disclose your personal information for other reasons, including if we believe such action is necessary to comply with the law or legal process, protect and defend our rights or property, protect the personal safety of our staff and facility or members of the general public.
